Our Booval GP's Welcome You!

For new patients – please download and print these two forms: New Patient Questionnaire (download form) and Privacy Consent Form (download form) for you to fill the form prior to your first practice visit. Please hand the forms to reception staff when you arrive for your first appointment. Full Privacy Statement is also available for you to read online or to download and read. Otherwise on arrival staff can provide you these forms.

How do appointments work?

Station Road Medical Centre operates by appointment so please contact the surgery on 3816 1155 where we will endeavour to provide you with a suitable appointment time on the same day or the day of your choice. If you have multiple issues to discuss with your doctor please advise the staff on booking your appointment and we can allocate the appropriate time to assist your doctor in giving you the time your concerns deserve.

You can also book online!

Kindly advise staff if you are unable to attend your appointment for any reason as we can then allocate that time slot to another patient in need of medical attention.

Bulk Billing Policy

PLEASE NOTE: FEE INCREASE FROM 1ST SEPTEMBER 2024

PLEASE NOTE OUR BILLING AND CANCELLATION POLICY

As of 1st November 2023, Station Road Medical Centre will be operating as a MIXED billing clinic, this means that some patients will incur an out-of-pocket cost for their appointments which is the difference between the practice’s private fee and the Medicare rebate.

We will continue to Bulk Bill for the following:

• All patients with a valid Medicare card including Aged and Disability Pensioners.

• DVA gold, white card holders will be bulk billed with the condition eligible to DVA (stated on the card).

• Health Care Card holders.

• Children under 16 years of age.

To be eligible for any service to be bulk billed you must have a valid Medicare Card and Concession Card.

For Private Patients

Full payment will be required on the day. We will transmit your claim directly to Medicare to assist you with a speedy reimbursement of your rebate, which will be directly deposited into your bank account.  Eligible patients have the option to claim an instant rebate.  For further details of current fees and claiming options, please contact our friendly reception team. We accept Eftpos, Credit Card or Cash.

Private Billing services for all Patients

Pre-employment Medicals, Insurance and Solicitor Reports, Commercial Driver’s License Medicals are privately billed and are not covered by Medicare. You will be advised by your doctor or our reception team about the applicable fee.

Cancellation Policy

Not attending appointments deprives other patients from seeking health care.
Please be advised, if you do not attend 2 or more appointments which are not cancelled a minimum of 2 hours before your appointment time, you will incur a $50 administration fee.
Our aim is not to collect fees but to open up otherwise booked appointments and make these available for patients requiring urgent assistance. This enables us to provide timely health care for new and ongoing patients.

Patients without a valid Medicare Card

If you are not eligible for a Medicare Card or your Medicare has expired, you will be privately billed. .

Please note: There may be a reduced fee for patients without a valid Medicare card, but this is at the treating Doctor’s discretion.
